This form has TWO (2) SECTIONS:

Section 1: Pay Online Order

Section 2:  Send Money to School Order

Once you enter your order information, scroll to the bottom of

this form.  Enter your e-mail address and click [SUBMIT] at the

bottom.

After you submit your order, you will be required to enter your

credit card information for any items chosen in the Pay Online

Order section.  Credit Card orders will be charged an additional

small convenience fee.

                                SECTION 1:  Pay Online Order

You may order multiple items and sizes.  Enter the QTY beside each SIZE you wish to order.

Then scroll to the bottom, enter your e-mail address, and click [SUBMIT].  You will be required

to enter your payment information to complete payment after you submit this order.

                     SECTION 2:  Send Money to School Order

If you DO NOT wish to pay online, use the section below to place your order. Once you enter your

order information, scroll to the bottom and enter your e-mail address.  You may submit

this form with no payment information.  If you choose this option, please send your money to

the school no later than Sept 24th.  Late payments will not be accepted and you shirt(s)

will not be ordered.

Money for Elementary orders should be sent to the Elementary office.  Money for 7th-12th orders

should be given to Mrs. Jana Jenkins, Mrs. Lena Tatum or Mrs. Callie Pinkerton or sent to the high school office. 

CASH or CHECK ONLY (payable to Bayou Academy).  NO VENMO.

Money sent to school MUST be CLEARLY MARKED with the student's name and indicate that the

payment is for Homecoming T-Shirt Orders.
